Divine Mobile Phlebotomy Services, LLC is responsible for the drawing and delivery of specimens to the lab for processing. DMPS strictly adheres to orders from Physicians or lab-labeled requisition forms for specimen collection. We do not provide medical advice, treatment, or diagnosis to any patient.
Divine Mobile Phlebotomy Services, LLC handles all sample deliveries to the lab. If a sample requires pickup by a courier service for transportation to the laboratory, DMPS assumes no responsibility for delays in arrival once the sample has been collected or picked up by the courier service.

Before scheduled appointments, Divine Mobile Phlebotomy Services requires accurate patient information, including patient ID, a copy of the insurance card (front and back), and lab orders. This information must be submitted to our office at least 48 hours prior to the appointment to ensure accuracy. Failure to provide the necessary information within the specified timeframe will result in the rescheduling of the appointment.
Divine Mobile Phlebotomy Services is not liable for any errors that occur at the laboratory. It is the responsibility of the laboratory to process specimens received from DMPS. In the event of a lab error, DMPS offers discounted rates for sample recollection. Errors made by DMPS will be rectified at no additional charge.

Any inquiries or issues regarding billing for tests collected through our service should be directed to the client's insurance company or the laboratory billing department.
Payment for services rendered is due the day before or the day of service.
DMPS operates as an independent convenience service and is not affiliated with any laboratories, doctor's offices, or hospitals. We do not have authorization to offer discounts for lab tests collected.
DMPS does not handle insurance billing; clients are responsible for submitting claims, with no guarantee of reimbursement.

Divine Mobile Phlebotomy Services, LLC will not disclose personal information to unauthorized individuals or entities without written authorization from the patient. All client information is kept private and confidential.
DMPS does not receive results on behalf of clients. Test results must be obtained directly from the physician to whom the results are sent.

Cancellation Policy:
In the event of a cancellation, a 24-hour notice must be provided to Divine Mobile Phlebotomy Services to allow for appropriate scheduling adjustments. A $25 inconvenience fee will be charged for all cancellations to cover administrative costs and rescheduling efforts. To request a rescheduling, please email admin1@dmpservice.site with your name, original appointment details, and address.
